Schedule your biometric screening today

The Hy-Vee wellness screening option has ended. If you haven’t completed a biometric screening yet and wish to be included in the early bird drawing, you may still complete a screening with your physician as long as you or your doctor fax the completed Physician Wellness Screening Results Form by Sept. 30.

The online registration form for on campus wellness screenings with UnityPoint is open. To guarantee a slot, register early. The screenings will be held between 6:30 and 10:45 a.m. in the lower level of the Olmsted Center on Oct. 4-6, 9, 11, 13, 17, and 19.  To schedule a screening, log in with the username DRAKE and password SCREEN (in all caps).

If you registered for an on-campus screening but don’t remember your appointment date and time, check your received, spam, or deleted email folders for an email confirmation from UnityPoint with the subject: Health Screen Confirmation. It would have been delivered to the email address you entered on your registration. Questions regarding the on-campus UnityPoint screenings should be sent to unitypointhealth.desmoines@gmail.com.

If you need to cancel an on-campus screening, send an email to jennifer.umphfleet@unitypoint.org with your name, date, and time of appointment you wish to cancel. You will need to re-register online for a new appointment. Changes can be made up to 48 hours prior to the appointment, if slots are still available.

If you have already completed your biometric screening, don’t forget to complete your online wellness assessment on wellmark.com by Nov. 17.

Complete your wellness screening now for a chance to win a prize

If you are on Drake’s health plan, you can save approximately $300 in 2018 by simply completing a biometrics screening and an online wellness assessment this year. Think what you could do with an extra $300. In addition, if you complete your screening and the online assessment at www.wellmark.com by Sep. 30, your name will be placed in a drawing for a chance to win your choice of a prize from an online wellness catalog.

There are three screening options:

  1. Complete a screening with your physician and have your doctor fill out a Physician Wellness Screening Results Form. This option is available through Aug. 31.
  2. Complete your screening at a participating Hy-Vee. This option is available through Sep. 15.
  3. Complete your screening on campus through UnityPoint Health. The on campus dates are Oct. 4–6, 9, 11, 13, 17, and 19. Online registration opens Sep. 1.

Faculty and staff can select the option that works best for them. However, it’s important to note that there are a limited number of spaces available for the on campus screenings, so employees are encouraged to complete their screening with their physician or at a participating Hy-Vee.

Wellness Screenings

Don’t forget to get complete your wellness screening. Those on Drake’s health plan can save approximately $300 in 2018 by completing a biometric screening and online wellness assessment. Choose one of the following options to complete your screening:

  1. Complete a physical (with bloodwork) with your physician by Aug. 31. Your physician’s office must complete, sign, and submit a biometric screening form by Sep. 30.
  2. Complete your screening at a participating Hy-Vee location July 1 through Sep. 15.
  3. Complete your screening on campus at the Olmsted Center through UnityPoint Health between 6:30 and 10:45 a.m. on one of the following dates: Oct. 4-6, 9, 11, 13, 17, or 19. You can schedule your appointment online starting Sep. 1.
